Office visits are scheduled by appointment only. Please bring the completed New Patient Information form and the New Patient Questionnaire to your first appointment. These forms are now available for download on the Home Page. Please also bring any pertinent information including diagnostic test reports and records from other doctors.

Cancellation Policy: If you are unable to keep your initial appointment, please notify us at least 2 business days in advance to avoid a late cancellation charge. For regular follow-up appointments, please notify us at least 1 business day in advance to avoid a late cancellation charge.

Monday:	8:30 am to 5 pm
Tuesday:	8:30 am to 3:30 pm
Wednesday:	8:30 am to 4:30 pm
Thursday:	8:30 am to 6 pm
Friday:	8:30 am to 3:00 pm (one Friday a month office closed)
