  • Our office building, 2500, is the first on the left.

Coming from the South:

  • Take the Wexford exit (73) on I-79.

Office Visits

Appointments are required for all office visits. To ensure a smooth first appointment, please bring the completed New Patient Information form and the New Patient Questionnaire, which can be downloaded from our Home Page. Additionally, bring any relevant information, including diagnostic test reports and records from other healthcare providers.

Cancellation Policy

If you are unable to attend your initial appointment, kindly notify us at least 2 business days in advance to avoid incurring a late cancellation fee. For regular follow-up visits, please inform us at least 1 business day ahead to prevent charges.

Fee Structure

Anticipated charges for office visits will be discussed with you during your initial appointment.

Insurance Policy

To ensure the highest quality of care, we do not participate in any health insurance plans. Payment is required at the time of service, and you will receive a receipt that can be submitted to your insurance carrier for reimbursement. Please note that reimbursement is not guaranteed, so we recommend checking with your insurance provider for specific coverage details.

Case Restrictions

Please be advised that our office does not accept Worker's Compensation and active litigation cases.
